Full medical coverage vs higher base salary โ€” how I made the decision

Faced this exactly: Offer A was RM8,500 base with full medical (hospitalisation RM150k, panel, dental, optical for self and spouse). Offer B was RM9,800 base with basic panel clinic card only.

RM1,300/month difference = RM15,600/year more in salary.

A private health insurance policy covering what Offer A provides would cost approximately RM4,000โ€“8,000/year for myself and my spouse. So the real take-home difference narrows to RM7,600โ€“11,600/year in Offer B's favour.

But: the employer-provided coverage in Offer A typically has no exclusion for pre-existing conditions, which private insurance does. And I have a pre-existing condition.

I took Offer A. The certainty of coverage mattered more than the salary gap after I ran the actual numbers.

Nomad visa is still unclear for Malaysia-based workers on foreign companies. LHDN and immigration rules conflict. Consult a tax lawyer.

DE Rantau is a good initiative but awareness is low. The co-working spaces attached to it are genuinely nice.

The tax treaty issue when working cross-border is the hidden complication. If you're a Malaysian working remotely for a Singapore company, you may owe taxes in both countries depending on structure.
