Poor work life balance. Too much barking about employee well-being and loads of BS programs & surveys for it, but none of that helps you or makes your life within the company any better. The company needs to go back to the drawing board & seriously work on identifying bad actors who are responsible for this kind of culture and get out of the bubble that their current surveys/its follow-up and loads of other well-being program are doing anything of value.
